Job summary:


The Life Enrichment Lead Aide will assist in planning, coordinating, and implementing therapeutic activity programs in our Skilled Nursing Facility on a daily basis to meet the individual resident's needs and interests. They will assess resident's activity participation on an on-going basis to monitor the effectiveness of the program.

Essential Responsibilities:


  • Plan, compile, and distribute a monthly activity calendar that reflects the needs and interests of the residents on the respective neighborhood.
  • Lead planned activities (usually 2-3 per day), using materials and equipment provided.
  • Provide one-to-one visits with residents as scheduled and keep a record of those visits. These visits may include reminiscing, sensory stimulation, socialization, and educational activities.
  • Attend all mandatory in-services, training, meetings, and seminars as directed by supervisor.
  • Attend weekly life enrichment staff and care plan meetings.
  • Maintain bulletin board(s) on respective neighborhood and ensure that information is current, accurate and organized.
  • Encourage residents to participate in activities and/or events by informing them daily of recreational opportunities.
  • Escort/transport residents in wheelchairs to and from events and activities.
  • Complete all relevant documentation (MDS interviews, progress notes, care plan notes, initial assessments, and daily charting) in an accurate and timely fashion.
  • Act as an advocate and observe resident rights at all times by maintaining confidentiality and privacy of personal resident information.
  • Work in collaboration with nursing and other departments to achieve team-based quality results.
  • Assess all new admissions to the facility.
  • Maintain an activity care plan for all residents and ensure that activity goals are met.
  • Coordinate with the Director of Life Enrichment for special events as needed.
  • Report to the Director of Life Enrichment regarding any changes to the resident's health and participation.


Requirements


  • High School Diploma (or equivalent)
  • Must have valid driver's license and ability to drive 14-passenger bus.
  • Perform the physical requirements of the position which include standing, bending, and lifting.
  • Work well with others; treat residents, co-workers, and staff with care and respect.
  • Protect resident confidentiality by not sharing information.
  • Adhere to all Wesbury personnel policies as they apply to the workplace.
